I have AT&T LTE and it's no faster than their HSPA+ (i.e. the "layer" between 3G and 4G). You're still mainly looking at around 5Mb/s, which isn't slow but is a far cry from the theoretical double digit Mb/s speeds that LTE is supposed to have. I've used it downtown mpls and several suburbs with similar results.

"Covered" is a bit of a stretch. Maybe if you lump them all together you could say that. Verizon probably has the broadest coverage. As an AT&T customer (forever) their LTE build out is pretty spotty. I live in Wright county, on a State Highway, and I get 1 bar at my house...I will give them until my contract is over...if no improvement, Verizon here I come.
