Less medicine for overwhelmed patients

Whenever there are medication issues with patients, a pharmacist should be involved in deciding which medications are appropriate, consulting the patient and taking patients off medications. Pharmacists are the medication expert on the healthcare team and many work in clinics and hospitals directly with physicians and patients to prevent and resolve drug-related problems.
