This service was an absolute godsend for my mother and I when she, living alone on a farm in ND was struck down with a stroke and we suddenly had to navigate moving her to MN for the immediate acute and ongoing care. Questions about Medicare, health insurance, nursing care, in home services, financial management, transportation, social opportunities, counseling, support for caregivers, nothing was out of bounds, and all offered with patience, respect, and compassion by the staff. Those people saved MY life. The most valuable state funded service!
