State certifies 11 health care homes

The step in changing the state's care system is intended to improve care and reduce hospitalizations and costs for people with chronic illnesses.

Given what it costs to go in for a clinic visit it appears that this might buy less than 15 minutes per month of care coordination. If there is a long list of requirements from the State in order to be certified, how is this making the system more affordable?
